INSTRUCTIONS

The present report refers to the electrical/electronic installation of the vessel and reflects the condition of every individual machinery item and component. After the inspection fill the “YES” or the “NO” option box accordingly. If further remarks should be made please, make use of the free paper space. Please bear in mind that “YES” means that the machinery/equipment is in good condition operating satisfactorily, while “NO” discloses a problematic or an unsmooth situation. Do not forget to make notes regarding the manufacturer and the type of machinery. The leaflet should be filled every six months and sent to Athens' Office. Special attention should be given to Safety Devices of the D/Gs, Boilers and M/E. In case of safety device malfunction and due to vessel's operations (or other reason) it must be set off, in order to carry on, (either electrically using a short circuit, or mechanically) the Office in Athens should be informed immediately.
