Instructions

Students residing in another munici-
pality because of their education
can retain their original domicile. A
request to that effect should be entered
in the form under Comments. A
certificate from the school in question
must be enclosed.

A special form must be filled in, signed
by both cohabiting partners.

Cohabiting partners with children
must submit certification of their
breakup from the District Commissioner
concerned; otherwise the notification
of change of address will not be taken
into account.

The notification must be accompanied
by a written consent from the child´s
custodian/custodians or person with
whom the child was last registered.

If a divorce is planned, this should
be mentioned in Comments. In the
event of a separated couple taking up a
joint residence again, this should be
mentioned under Comments.
